Time Away Forms
Students who wish to suspend their graduate study should apply for a leave of absence by filling out the appropriate Time Away form in Workday Student. Detailed step-by-step instructions are available. A student’s application for a leave of absence must be endorsed by the degree program and then approved by the Office of Graduate Studies. For more information, please refer to the A&S OGS Policies and Procedures page.
Leaves will not be granted for less than one semester or for more than one calendar year. If a student requests a leave of absence beyond one year, the student’s status will be reviewed to determine whether an extended leave will be approved.
All international students should consult with the Office of International Students & Scholars before applying for a leave of absence.
A leave, with its loss of student status, may have implications in many areas which should be considered. These may include:
- Student Health Insurance
- Student loans and loan deferment
- Legal status for F-1 and J-1 student visas
- Student funding (fellowships, etc.)
- The right to rent University-owned housing
- Access to University facilities
Students requiring a leave of absence for family/dependent care, military service or other personal reasons should fill out the Leave of Absence form.
Students needing a medical leave of absence should fill out the Medical Leave of Absence form after consulting with the appropriate Health Services office their campus.
If students will need to use Olin Library during the leave, this can be arranged by having their department send an email to the Circulation Library requesting their access to the library for the period of the leave.
At the end of any leave of absence, a student is reinstated into the Office Graduate of Studies under the conditions prevailing at the time the leave was granted.
All students returning from leave or medical leave must submit a Request for Reinstatement form to their department or program.
There will be no reinstatement fee for students returning from approved leaves.
Medical Leaves of Absence (MLOA) are granted and regarded as approved interruptions of a student’s program. The university recognizes that medical and mental health conditions can interfere with a student’s academic success and personal safety. All medical leaves must be recommended by WashU Cares (WUC) for Danforth-based students. The WUC detailed process for requesting an MLOA can be found here. Once a student has secured the recommendation, they must follow the Workday process for requesting time away.
To return from an MLOA, students must first follow the WUC process, which includes a deadline and window for reinstatement application.
All students returning from a leave of absence must be formally reinstated into their department/program. Please review the Office of the University Registrar’s information on reinstatement to determine your eligibility for reinstatement and complete a reinstatement request form. Students returning from an MLOA must follow the WUC process.
Students who are withdrawing from WashU with no intention of returning must submit the appropriate Time Away form in Workday Student. Detailed step-by-step instructions are available.
Registration and Enrollment Forms
To request Curricular Practical Training (CPT), please contact the Office of International Students & Scholars (OISS) to obtain a CPT Application Form. Follow the instructions on OISS’s “Curricular Practical Training (CPT)” webpage to complete the form and obtain departmental approval.
Once the form is complete, your department should submit it to the Office of Graduate Studies via the ArtSci Portal.
Requests must be submitted no later than the first day of the semester, though earlier submissions are encouraged. Registration for the appropriate course(s) must be complete by Add/Drop.
Before registering for a Mentored Professional Experience (MPE), students must fill out Parts I and II of the MPE Registration and Evaluation Form and receive approval from their MPE site mentor, the Instructor of Record for ASGS 8120, and their DGS.
After receiving departmental/program approval, the department/program must submit the MPE Registration and Evaluation Form to the Office of Graduate Studies (OGS) via the ArtSci Portal by the first day of the semester at the latest. Once approved, the student must register for ASGS 8120.
Upon completing the MPE, students must complete Part III of the MPE Registration and Evaluation Form with their site advisor and submit the completed form to the Instructor of Record listed for the course.
Mentored Professional Experience (MPE) Registration and Evaluation Form
Before beginning a Mentored Teaching Experience (MTE), students must register for the appropriate MTE course (ASGS 8005, 8010, 8015, or 8020) and complete the Mentoring Plan Form. Students must submit the Mentoring Plan Form to the Instructor of Record for the course.
Upon completing the MTE, the Instructor of Record/faculty mentor for the course must complete the MTE Evaluation Form and determine the final grade for the course.
The department/program should retain the MTE Evaluation Form for record-keeping.

Sponsored by WashU, the purpose of the Child Daycare Subsidy is to help PhD student families meet the costs of child daycare while they pursue their studies. Please review the Office of the Provost’s Child Day Care Subsidy page to review eligibility guidelines and to apply.
Full-time PhD students may request a New Child Leave to assume care for a new child. They should maintain their full-time student status. Please see the University Bulletin for additional information.
Once the form is complete, your department should submit it to the Office of Graduate Studies via the ArtSci Portal.
